Youth Free Try Out Waiver

Your kids may be dropped off anytime after 11am and must be picked up by 5:15pm. The try out starts at 3pm officially, but there will be a meet and greet and small group lessons from 2-3pm.

I, the undersigned parent or legal guardian of the minor named below, hereby give my consent for my child to participate in a free try out at Gee Sports Corporation. I understand that this activity involves physical exertion and that there are inherent risks involved in participating in sports.

In consideration of my child being allowed to participate in the free try out, I, on behalf of myself, my child, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Gee Sports Corporation, its directors, officers, employees, agents, volunteers, and sponsors (collectively referred to as “Releasees”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, or to any property belonging to me or my child, while participating in the free try out, or while on or upon the premises where the try out is being conducted.

I understand that this waiver and release of liability includes, but is not limited to, any claims arising from the negligence or fault of the Releasees and covers bodily injury, property damage, or wrongful death arising from any activities related to the free try out.

I acknowledge that I have read and understand this waiver and release of liability, and I voluntarily sign it, intending to be legally bound by its terms. I also certify that I am the parent or legal guardian of the minor named below and that I have the authority to enter into this agreement on his/her behalf.
